$0-$40,000 $1,350 per month $0-$100,000 $850 per month
$40,001-and up 3% of insurance collections $100,001-$200,000 $1,250 per month
$200,001-$250,000 $1,650. per month
Insurance Verification: $250,001-and up $1,850 per month 0-50 $750
51-100 $1050
101-and up $1250
*Insurance Billing Rates are based on total monthly insurance collections.
*Patient Billing Rates are based on total office collections.
*Stamps and Collection Letter reimbursement in addition to monthly rates
